TPP automatically fetches the latest proxy information and then it applies some basic changes to your hosts file without losing your custom rules.
So there is no need to backup your hosts file, don't worry.
Currently there 4 options. 1 applies the patch. 2 removes the patch. 3 updates your hosts file with the latest fetched proxy information. And 4 is just an about box. 

Important: Before running the app, please be sure you have any security application disabled so TPP can write to your hosts file!
